Statistics regarding the most frequent poisons, their appearance and symptomatology, and antidotes are useful to clinical practice and education to be available while carrying out successful and timely treatment of cases of poisoning. Mechanism of Action of Poisons and Their Types<\/p>\n<p>A poison can cause an intoxication by acting on any of the enormous array of materials that range from chemicals and drugs to biologic toxins. Pralidoxime (PAM) also reactivates acetylcholinesterase if administered in a timely manner.<\/li>\n<li>Location of Instructional Focus &#8211; Atropine to reverse muscarinic, and pralidoxime to reverse muscarinic and nicotinic.<\/li>\n<\/ul>\n<ol start=\"3\">\n<li><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK507796\/#:~:text=Cyanide%20toxicity%20is%20a%20rare,individual%20murders%2C%20and%20chemical%20warfare.\" target=\"_blank\" rel=\"noopener\"><strong>Cyanide Poisoning<\/strong><\/a><\/li>\n<\/ol>\n<ul>\n<li>Pathophysiology &#8211; Cyanide is fixed by cytochrome c oxidase in mitochondria and prevents tissue hypoxia and cellular respiration.<\/li>\n<li>Hydroxocobalamin and sodium thiosulfate. Sodium thiosulfate contributes sulfur groups in order to combat the toxic effect of cyanide by oxidizing the toxic cyanide to the less toxic form thiocyanate to be excreted from the body by urine. Cyanide is bound by hydroxocobalamin, and cyanocobalamin is excreted from the body by urine.<\/li>\n<li>Exam Key Point to Remember &#8211; Headache, dizziness, confusion, and seizures are typical of cyanide poisoning.<\/li>\n<\/ul>\n<ol start=\"4\">\n<li><a href=\"https:\/\/medlineplus.gov\/ency\/article\/000165.htm\" target=\"_blank\" rel=\"noopener\"><strong>Digoxin Toxicity<\/strong><\/a><\/li>\n<\/ol>\n<ul>\n<li>Mechanism: Inhibition of the Na+\/K+ ATPase pump by digoxin, increase in intracellular calcium with resulting arrhythmias.<\/li>\n<li>Antidote: digoxin-specific antibody (sheep-derived Digibind).<\/li>\n<li>Exam Key Point &#8211; Digitalis toxicity is classically most commonly presented with visual alterations (e.g., yellow vision), nausea, and arrhythmias.<\/li>\n<\/ul>\n<ol start=\"5\">\n<li><a href=\"https:\/\/www.msdmanuals.com\/home\/injuries-and-poisoning\/poisoning\/iron-poisoning\" target=\"_blank\" rel=\"noopener\"><strong>Iron Poisoning<\/strong><\/a><\/li>\n<\/ol>\n<ul>\n<li>Mechanism: Oxidative damage to cellular membranes induced by iron, secondary gut damage, shock, and hepatotoxicity.<\/li>\n<li>Antidote: Deferoxamine\u2014Binds free iron to be excreted by the kidneys.<\/li>\n<li>Exam Key Point &#8211; Acute iron poisoning requires deferoxamine, causing reddish-brown urine coloration.<\/li>\n<\/ul>\n<ol start=\"6\">\n<li><a href=\"https:\/\/en.wikipedia.org\/wiki\/Methanol_toxicity\" target=\"_blank\" rel=\"noopener\"><strong>Methanol Poisoning<\/strong><\/a><\/li>\n<\/ol>\n<ul>\n<li>Pathophysiology &#8211; Metabolized to formaldehyde, then formic acid, the latter retinotoxic and causing blindness and metabolic acidosis.<\/li>\n<li>Antidote &#8211; Fomepizole or Ethanol\u2014Both inhibitors of alcohol dehydrogenase that prevent methanol metabolism to toxic metabolites.<\/li>\n<li>Important Point of Inspection &#8211; Blindness, vomiting, nausea, dizziness, and headache are due to methanol poisoning.<\/li>\n<\/ul>\n<ol start=\"7\">\n<li><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK482238\/#:~:text=The%20classic%20presentation%20in%20patients,ataxia%2C%20and%20altered%20mental%20status.\" target=\"_blank\" rel=\"noopener\"><strong>Benzodiazepine Overdose<\/strong><\/a><\/li>\n<\/ol>\n<ul>\n<li>Mechanism &#8211; Benzodiazepines cause CNS depression by increasing neurotransmitter GABA action on the receptor GABA-A.<\/li>\n<li>Antidote\u2014Flumazenil: It is a GABA receptor antagonist that will reverse benzodiazepine action.<\/li>\n<li>Critical Point of Exam &#8211; Use caution while administering flumazenil in seizure patients or tricyclic antidepressants.<\/li>\n<\/ul>\n<ol start=\"8\">\n<li><a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/carbon-monoxide\/symptoms-causes\/syc-20370642\" target=\"_blank\" rel=\"noopener\"><strong>Carbon Monoxide Poisoning<\/strong><\/a><\/li>\n<\/ol>\n<ul>\n<li>Mechanism\u2014Carbon monoxide is attached much more strongly than oxygen to hemoglobin to carboxyhemoglobin and disrupts tissue oxygenation.<\/li>\n<li>Antidote: 100% Oxygen\u2014By non-rebreather mask that displaces carbon monoxide from hemoglobin and enhances carboxyhemoglobin clearance.<\/li>\n<li>Rare Exam Point Reduction &#8211; To be started at the earliest in cases of suspicion, and hyperbaric oxygen therapy may be considered in complex cases.<\/li>\n<\/ul>\n<ol start=\"9\">\n<li><a href=\"https:\/\/medlineplus.gov\/ency\/article\/000774.htm#:~:text=Symptoms%20may%20include%20nausea%2C%20vomiting,smell%20alcohol%20on%20their%20breath.\" target=\"_blank\" rel=\"noopener\"><strong>Ethylene Glycol Poisoning<\/strong><\/a><\/li>\n<\/ol>\n<ul>\n<li>Mechanism &#8211; Metabolized to glycolic acid and oxalic acid and causes renal failure and metabolic acidosis.<\/li>\n<li>Antidote: Fomepizole or ethanol\u2014both alcohol dehydrogenase inhibitors\u2014thus prevent metabolism of ethylene glycol to its toxic metabolites.<\/li>\n<li>Point of Examination of Importance &#8211; Ethylene glycol toxicity is most often described as CNS depression, hypocalcemia, and renal failure.<\/li>\n<\/ul>\n<ol start=\"10\">\n<li><a href=\"https:\/\/accessmedicine.mhmedical.com\/content.aspx?bookid=2284&amp;sectionid=248384444\" target=\"_blank\" rel=\"noopener\"><strong>Heparin overdose<\/strong><\/a><\/li>\n<\/ol>\n<ul>\n<li>Pathophysiology: An anticoagulant, heparin, binds to and inhibits thrombin and factor Xa activity. Bleeding heparin.<\/li>\n<li>Antidote: Protamine Sulfate\u2014Binds to and antagonizes heparin&#8217;s anticoagulant effect.<\/li>\n<li>Point of Examination of Importance &#8211; Protamine sulfate must be administered with caution, as it will result in hypotension with rapid intravenous injection.<\/li>\n<\/ul>\n<p>The students should be familiar with the poisons, their presentation, and the antidotes of the NEET PG and FMGE exams. The best way to do that is to use common poisons, their pathophysiology and presentation, and, most importantly, the antidotes and management.
